Questionnaires allow you to survey students and other course participants using a range of questions. Questionnaires differ from Moodle Quizzes as Questionnaires are only used to gather data instead of assessing students. They are useful for receiving course feedback, getting to know students, and a variety of other non-evaluative activities.
Creating a Questionnaire
Before you can add content to your questionnaire, you must first create a questionnaire activity. This begins the same way as adding any other activity:
- Toggle Edit Mode from the top right of the course page
- Click Add an Activity
- Select Questionnaire from the grid of options
- Give the activity a Name and Description
- Choose which settings you would like to apply to the activity
- Some helpful settings include importing questions from existing questionnaires, allowing recurring responses, anonymizing responses, and more
- Once your settings are finalized, click Save and Display > Add questions
- From the dropdown menu, select which question type you would like to add. Question types include:
- Check Boxes
- Date
- Dropdown Menu
- Essay Box
- Label
- Numeric
- Radio Buttons
- Rate (scale 1 - 5)
- Slider
- Text Box
- Yes/No
Configuring Questions
Once your question type is selected, you will be brought to a new page that displays settings for your question. Each question type has unique settings. Since it is a popular question type, we will walk through Essay Box settings:
Note: For questions that take answers longer than a few words, we recommend choosing the Essay option over the Text Box.
- Type in a Question Name
- This is not the question itself, it is often a numerical name, e.g. ‘Question 1’
- Choose whether or not a Response is required
- Choose which type of Response format respondents use to answer
- Plain text provides a basic textbox to type in
- HTML editor allows students to format their text (bold, italicize, bullet points, etc.)
- Select an Input box size
- This changes how large the textbox appears on the screen. Adjusting this setting will not affect how much or how little a respondent can type
- Type your question in the box labeled Question Text
- Once all settings are correct, click Save changes
New questions can also be added by editing existing questions. This is particularly useful when your question’s settings greatly diverge from the default settings.
- Clicked the gear (⚙) icon of an existing question
- Change the Question Name and the Question Text
- At the bottom of the page, click Save as New Question
This way, you can quickly add questions with your desired settings without having to reselect all of your preferences each time.
